Certain pieces of information must be contained in the application form to make it an integral part of the contract. These are as follows;  a.        The name of the insurance company must be prominently displayed on the application form.  b.        The name of the product.  c.        Because of the increasing number of fraudulent practices in the insurance industry, there should be a prominently displayed “fraud warning” which informs the applicant that knowingly presenting false information is a crime.  d.        If an agent assists the applicant in filing out the application form, the name and licence number of the agent should appear on the application form.
