Minerals and Mining (Licensing) Regulations 2012 (LI 2176)

Regulation 166. (1)  Where  an application  for surrender  is approved,  the Mineral Titles   Department  of the  Commission shall  record   the  details   of the  approval in  the General Register, and  if  applicable replace  the provisional  polygon on the cadastral  map with a polygon corresponding to the area surrendered,  and assign appropriate  codes.(2) Where  an application  for surrender  is approved,  the rights of the  holder  shall  cease without  affecting the  liabilities  or obligations incurred by the holder or the holder's agent in relation to the licence prior to the date of the surrender, and the title holder is not entitled to a refund of any fees paid.(3)  Where  the surrender  is in respect of the whole  area covered by the licence, the licence shall be terminated.

Procedures to Follow

Submit Your Application
Once you have filled and gathered all the required documents you will have to submit it at the district office.
It takes 5 days to review the application. The next 5 days is for notification of errors.
Submission To Mineral Titles Department (MTD)
10 days after registration, the district office shall submit the application to MTD.
MTD review the application in 10 days and records it in the priority register 
MTD Publishes In The Gazette
15 days after recording, MTD publishes in the gazette, notify chiefs, traditional authorities etc. through the Commission's District Offices. 
Application Dossier
Within
ten (10) days after publication, District Office shall submit application dossier to MTD for appropriate recommendation to Minister
Minister Takes Decision
Once you have completed these steps you are ready to start your business.
You need to renew your license every three (3) years.
Approval / Rejected
21 days notification to applicant and Fees payable if approved.
Reasons are given if rejected. 
Applicable Fees
Within 60 days, applicant pays applicable fees and gives notice of acceptance of the grant to Minister and Commission in writing. 
License
Minister on proof of payment of applicable fees, issues licence to applicant within 30 days after the date of acceptance of the grant.
