Minerals and Mining (Licensing) Regulations 2012 (LI 2176)

Regulation 217. (1) A holder  of a small scale mining licence may, not later than ninety days before the expiration  of the term of the licence, apply to the District  Office of the  Commission,   as set out  in Form  Seventy  of  the First Schedule for an extension  of the term of the licence in respect of all or part of the area subject to the licence.(2) The application shall include(a) a certified copy of the mining  licence;(b) the cadastral co-ordinates  delineating  the area subject to the application  for extension in the case of reduction  or enlargement  in the perimeter  of the original  polygon;(c)  annual reports of the  applicant   for  the  last  two  years, including  audited  accounts  and production  statistics, where the applicant  is a body corporate;(d) production  statistics and  particulars of sales, including details  of buyers;  and(e)  evidence of payment  of the applicable  fees.

Procedures to Follow

Submit Your Application
Once you have filled and gathered all the required documents you will have to submit it at the district office.
It takes 5 days to review the application. The next 5 days is for notification of errors.
Submission To Mineral Titles Department (MTD)
10 days after registration, the district office shall submit the application to MTD.
MTD review the application in 10 days and records it in the priority register 
MTD Publishes In The Gazette
15 days after recording, MTD publishes in the gazette, notify chiefs, traditional authorities etc. through the Commission's District Offices. 
Application Dossier
Within
ten (10) days after publication, District Office shall submit application dossier to MTD for appropriate recommendation to Minister
Minister Takes Decision
Once you have completed these steps you are ready to start your business.
You need to renew your license every three (3) years.
Approval / Rejected
21 days notification to applicant and Fees payable if approved.
Reasons are given if rejected. 
Applicable Fees
Within 60 days, applicant pays applicable fees and gives notice of acceptance of the grant to Minister and Commission in writing. 
License
Minister on proof of payment of applicable fees, issues licence to applicant within 30 days after the date of acceptance of the grant.
