Basic National Social Security Scheme Regulations, 2011 (L.I. 1989)

Regulation 29 (1) An application for survivors lump sum benefit shall be made on the appropriate form. (2) The applicant shall attach to the application, (a) a certificate or other evidence of death of the member, and (b) letters of administration except where the applicant has been nominated as a dependant for payment of a benefit. (3) The Director-General may require an affidavit or statutory declaration or any other evidence establishing the identity of the applicant and relationship to the deceased where the need arises.
